Subsidies are incentive funds given to employees to offset the cost of insurance. When calculating costs of coverage subsidies will be subtracted from the base rate.
Wellness Subsidy – Employee and/or Spouse: An employee and/or Spouse will receive an offset to their premium based on completion of the Wellness Program within the required timeframe.
Clinic Access Subsidy: If you are a remote employee living outside of the below counties in Kentucky and Indiana and are enrolled in an Appriss Medical benefit plan, you will receive an additional $84.00/month* subsidy.
Technical Support Specialist/Escalation Specialist Subsidy: If you are a Technical Support Specialist/Escalation Specialist enrolled in an Appriss Medical benefit plan, you will receive an additional $50.00/month* subsidy.
A surcharge is an additional charge to employees. When calculating costs of coverage surcharges will be added to the base rate.
Tobacco Surcharge – Employee and/or Spouse: An employee and/or Spouse will receive an additional surcharge to their premium, based on failure to complete the Tobacco Affidavit and/or Reasonable Alternative Content within the required timeframe.
*All benefits and subsidies are included in the first two paychecks of each month only. Divide the amount by 2 for the per paycheck amount. PLEASE NOTE: Subsidies will show in earnings, Surcharges will show in deductions. Your medical deductions will show as the base rates of the plan. The subsidies and surcharges will not reduce or increase the premium you see on your paycheck.
