Life Insurance

Employer-paid basic and employee-paid optional life are provided to Appriss by The Hartford. Outside of the new hire enrollment period, an Evidence of Insurability (EOI) will be required for all coverage increases and electing for the first time outside of the initial enrollment period. This benefit can only be elected/changed during new hire enrollment (31 days from date of hire to enroll), a qualifying life event, or annual open enrollment.

Life and Accidental Death & Dismemberment(AD&D) Insurance Benefits

You have options for both company-paid and optional employee-paid life and AD&D insurance.

Free Basic Employee Life and AD&D Insurance

Appriss offers free Life and AD&D coverage for full-time regular employees at a standard amount of $50,000. All you have to do is assign a beneficiary in Workday. Your coverage is reduced to 65% at age 65 and 50% at age 70. Coverage is discontinued at termination of employment or retirement.

Optional Employee and Dependent Life & AD&D Insurance

You must elect optional life coverage for yourself in order to cover your spouse and/or children. Your premiums for employee and spouse life are based on covered employee and/or spouse’s age. Benefits cease at termination or retirement.

Employee Optional Life

You may request up to five times your annual salary or a maximum of $500,000 (in $10,000 increments), whichever is less. The guaranteed issue amount is three times your annual salary up to a maximum of $200,000. Your coverage is reduced to 65% at age 65 and 50% at age 70. Coverage is discontinued at termination of employment or retirement.

Spouse Optional Life

Spouses are eligible for half of employee optional coverage or a maximum of $100,000 (in $5,000 increments), whichever is less. The guaranteed issue amount for spouses is $30,000. Coverage is reduced to 65% when Spouse is age 65 and ends when employee turns 70.

Child(ren) Optional Life

Child(ren) Optional equals $10,000 for each eligible child up to age 19 years old (or 25 if a full-time student, unmarried, and dependent on employee for 50% or more financial support).

Evidence of Insurability

An Evidence of Insurability (EOI) is required if you are electing optional life for yourself or your spouse outside of your initial enrollment period or if you are increasing one of these amounts at a later date. The Hartford will reach out to you via email with the necessary workflow for you to complete.  They will then review and let you and Appriss know if you have been approved. Payroll deductions for the increased amount or new election will not be deducted until you are approved.
